To obtain Hearts in Blox Fruits, you need to head to an Island such as Chocolate and others and find NPCs and Mobs of your level.
Yes, the mobs or NPCs you are going to kill must be on the same level as yours. When you kill those NPCs and Mobs of your level, chances are high that you will get one or two Hearts when they are defeated.

For the unversed, it has a <40% chance to drop from an NPC. Once you have got enough Hearts by killing mobs or NPCs, you can use them in the Event Shop and purchase Fruits and other in-game accessories.
If you want to earn a lot of Hearts fast in Blox Fruits, I highly recommend Friend boost, which increases the number of Hearts in the NPC drops.
