To complete the Pain and Suffering trial in Blox Fruits, you need to teleport to the Third Sea. Once teleported, find and talk to an NPC named Crypt Master.
When you take a quest from the Crypt Master, you will have the door of the Turtle Mansion opened. When you get there, you will find two scrolls — Yama and Turshia.

To complete the Pain and Suffering trial, make sure to take the Yama scroll first. When you do so, you will be asked to complete three trials and Pain and Suffering is the first one.
In order to do the Pain and Suffering trial in Blox Fruits, you need to equip the Yama sword first. Once equipped, you need to take damage from an NPC.
This might take a while but you need to keep taking damage from an NPC until a message appears on the screen, “The scroll reacts to your achievements.”
When you see this message on the screen, it means the Pain and Suffering trial has been completed. Besides this message, you will get an Alucard Fragment as a reward.
