In Sakura Stand, you can obtain the Okarun Spec by completing two quests.
Fortunately, these quests do not have any level requirements which means anyone can complete them and get the Okarun Spec in the game.
However, to start these quests you need to go to the Cave and talk to the NPC called Turbo Granny.
This NPC is located inside the cave area. To reach there, head straight from your spawn location and then take a left turn.
After that, take a right turn and go straight. At the end of this road, you will see a tunnel which is a Cave.
Now, go closer to this tunnel and press E to interact. Once you interact with the caveman, it will teleport you to another location.
Here, you will see two NPCs. Now, go and talk to the NPC in front of you.
This is Turbo Granny. After interacting with this NPC, she will give you a quest.
To complete this quest, you need to collect 100 Souls from the Curses and Hollows.
To collect souls and complete this quest, you need to find the Curses and Hollows and kill them.
You need to do 100 times and collect 100 Souls.
After collecting the Souls, go back to the cave and this time interact with the NPC on your left.
The name of this NPC is Momo. After interacting with Momo, she will give you another quest.
Now, complete the quest given by Momo and you will obtain the Okarun Spec in the game.
